Case Management Nurse - Case Manager

Exclusive Order Typical Diagnosis: Case management Most common medications: All drips and cardiac meds Typical Nurse to Patient Ratio: We work on a team pairing system so it varies from 9 to 15 No floating Fast Paced and very dynamic environment Adult Management is the most important skill for this opening. This Case Manager will be expected to work through out the hospital in all units. Will work Weekend Rotation with Staff!

Required Qualifications

1. Good nursing skills and understanding of all UM function. Experience as discharge planner, case managers 2. basic computer skills to be able to place notes, make referral to Home Health and DME 3. Acute Care experience
